We are delighted to say that we have now created a digital version of the form to ensure that you can let us know of any concerns quickly and efficiently.
Reporting an incident helps us to understand the types of incidents occurring and identify trends. This helps to inform our guidance and enables us to address safety issues and concerns.
Our approach to incident reporting is based on learning from what went wrong and preventing similar incidents from happening in the future. It is not designed to point blame or result in negative consequences.
